I'm afraid this sad story will not be the last one we hear about.

The government department concerned, only goes by on what is written down on forms and not on reality.

This comes from experience, as having to deal with them, you fill like your hitting your head agains't a brick wall.

I notice their get out clause came into play.

"We had not received any complaint from them about their benefit claim".

Most likely they could not afford the postage to appeal.
