If you do have to send one ( or other pix/forms etc as well ).

Have you seen graphics files when they have been corrupted ? They quite often have great green square bars of colour through them totally obscuring large important portions of it. It could be recognised vaguely that there is a passport there but totally unusable, nothing can be read od seen. Supposedly caused by incomplete grahic data only laying down one component of the three needed for his screen/printer to do anything with.

scanner must be damaged, computer or software problem ( his of course !! )
Internet is broken, ..again !
